29 CFR 1910.28(b)(6)(i): The employer did not ensure that each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard: On or about September 3, 2025, September 4, 2025, September 5, 2025 and September 9, 2025, employees were exposed to unprotected fall hazards onto the drag conveyor through open sump holes in Grain Bin #48, Grain Bin #49 and Grain Bin #41, at the Dalton City, Illinois, facility.

29 CFR 1910.272(g)(1)(ii): Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not disconnected, locked out and tagged, block-off, or otherwise prevented from operating by other equally effective means or methods: On or about September 3, 2025, September 4, 2025, September 5, 2025 and September 9, 2025, employees were exposed to amputation hazards from the unguarded drag conveyor in Grain Bin #48, Grain Bin #49 and Grain Bin #51 thru open sump holes, and the employer did not deenergize and lockout, block-off, or prevent from operating all equipment which could create employee hazards, including the drag conveyor, at the Dalton City, Illinois, facility.
