29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) On or about 09/23/2025 throughout establishment; where employees perform servicing and/or maintenance activities such as, but not limited to: setting up presses, changing grinder wheels and replacing saw blades. The employer has not developed machine specific energy control procedures, nor has the employer trained to the authorized level or periodically reviewed employees performing covered operations where the employees were exposed to potentially hazardous energy sources and amputation hazards.  ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.217(e)(1)(i): The employer did not maintain a certification record of inspections which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the power press that was inspected.  a) On or about 09/25/2025 in the Welding Department, employee(s) who operated the power presses were exposed to a caught between hazard when the employer did not establish an inspection program which included a certification and record of the inspection, the date performed, signature of who performed the inspection and an identifier of which press was inspected.
