(a) On or about September 26, 2025, at 2901 Rocky Creek Dr., Waukesha, WI 53189, employees installing a roof on a residential home roof with a 10:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 17 feet 11 inches from the roof eve to the ground below on the back of the home.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. (a) On or about September 26, 2025, at 2901 Rocky Creek Dr., Waukesha, WI 53189, employees conducting roofing activities on a 10:12 pitch roof were not properly protected from falling to the lower level. The attachment point of the body harness was front and centered at the waist (belt area) and not located in the center of the wearer's back near shoulder level, or above the wearer's head as required. This exposed the employees to severe spinal and neck injuries.
