29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Worksite, town hall, concrete rounded wall: On or about 10/27/2025, employees were using an extension ladder to access a roof, approximately 24 feet above the ground. The side rails of the ladder extended approximately 20 inches above the landing zone.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Worksite, town hall, concrete rounded wall: On or about 10/27/2025, employees were replacing the roof of a commercial building, approximately 24 feet above the ground, and were not protected from falls after accessing the roof from a Werner extension ladder. Employees were not wearing the harnesses or lanyards as part of a personal fall arrest system. b) Worksite, town hall roof: On or about 10/27/2025, employees were replacing the roof of a commercial building, approximately 40 feet above the ground, and were not protected from falls. Employees were not wearing the harnesses or lanyards as part of a personal fall arrest system. JFB Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1635220, citation number 1, item 1, and was affirmed as a final order on 4/28/2023, with respect to a workplace located at 516 Thompkins Street, Syracuse, New York.
