29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Worksite, town hall, roof: On or about 10/27/2025, employees conducting roofing activities on a commercial building and the employer did not ensure the employees were trained to recognize fall hazards and procedures to follow to minimize fall hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Worksite, town hall, concrete rounded wall: On or about 10/27/2025, employees were using an extension ladder to access a roof, approximately 24 feet above the ground. The side rails of the ladder extended approximately 20 inches above the landing zone.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Worksite, town hall, concrete rounded wall: On or about 10/27/2025, employees were replacing the roof of a commercial building, approximately 24 feet above the ground, and were not protected from falls after accessing the roof from a Werner extension ladder.  Employees were not wearing the harnesses or lanyards as part of a personal fall arrest system.  b) Worksite, town hall roof: On or about 10/27/2025, employees were replacing the roof of a commercial building, approximately 40 feet above the ground, and were not protected from falls. Employees were not wearing the harnesses or lanyards as part of a personal fall arrest system.  JFB Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1635220, citation number 1, item 1, and was affirmed as a final order on 4/28/2023, with respect to a workplace located at 516 Thompkins Street, Syracuse, New York.
