At the drainage pond excavation development site, located to the Southwest of unmarked and unprepared temporary access roads from the Intersection of Welaunee Boulevard and Dempsey Mayo Road at 30.49 N Latitude, 84.19W Longitude, in Tallahassee, Florida: On or about November 5, 2025, the employer did not make prior arrangements for medical or emergency services to provide prompt medical attention to employees exposed to struck-by hazards from heavy equipment during site preparation and excavation operations.
