29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    Or in the alternative;    29 CFR 1910.28(b)(3)(i):  The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D):    a) On November 6, 2025, at University of Mary, Welder Library, 7500 University Dr, Bismarck, North Dakota, the employer failed to protect employees from injury in that, employees did not use personal fall arrest systems or other acceptable means of fall protection or guarding during the installation of skylight glass on the roof of University of St. Mary Welder Library. This condition exposed employees to a fall of approximately 14 feet.
