29 CFR 1910.147(f)(1)(ii): Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section; On or about October 28, 2025, at an establishment located at N5279 Co Rd G in Saint Cloud, Wisconsin, in the Cheese Production area, an employee was exposed to machine hazards while performing sanitation activities on the A1 Incline conveyor and was exposed to a caught-in hazard from the in-running pinch point between the belt and idler roller. The employer did not ensure that the employees implemented energy control application steps or equivalent protective measures prior to allowing them to clean conveyors with the equipment running and an employee reached into the moving equipment to remove a piece of cheese, which resulted in their shirt becoming entangled in the moving equipment. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. On or about October 13, 2025, at an establish located at N5279 Co Rd G in Saint Cloud, Wisconsin, in the River Brine Room, the employer did not provide written periodic inspection certifications for the lockout tag out audits on machines including but not limited to the 1A Horizontal conveyor and 1A Incline conveyor.
