29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  A) Bakery Kitchen:  The large floor Hobart mixer, used by employees to make dough for dinner rolls and whip egg whites to make chiffon, had rotating shafts and blades that were not guarded.  Employees were exposed to rotating parts hazards and caught between hazards.  B) Biba's Kitchen: A mid-sized Hobart mixer, used by employees to make mashed potatoes, had rotating shafts and blades that were not guarded.  Employees were exposed to rotating parts hazards and caught between hazards.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Engineering Shop Workstation: A Relocatable Power Tap that powered phone and battery charges was permanently attached to the workbench with zipties.  Employees were exposed to electrical hazards.  The Hale Koa Armed Forces Recreation Center was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1597198, citation number 2, item number 1 and was affirmed as a final order on 6/23/2022, with respect to a workplace located at the Hale Koa Armed Forces Recreation Center.

29 CFR  1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  Engineering Department hallway, near engineering employees' workstations: A knockout was missing from the middle of a 120 volt electrical switch box providing power relay to local outlets. Employees were exposed to electrical hazards.  The Hale Koa Armed Forces Recreation Center was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1597198, citation number 1, item number 6 and was affirmed as a final order on 6/23/2022, with respect to a workplace located at the Hale Koa Armed Forces Recreation Center.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  Laundry Room, outside the hallway to the Assistant Housekeeper's Office: A closed chain mounted across the opening in front of three electrical panel boxes limited access to the back two electrical panels.  Employees were exposed to electrical and fire hazards.
