29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. a) Body Shop: On or about November 18, 2025, the employer exposed employees to trip hazards in that, the employer failed to ensure that cords used to extension cords, equipment power cords and tools were not kept on the floor while employees were working/walking in the area. b) Car Wash Bay: On or about November 18, 2025, the employer exposed employees to trip hazards in that, the employer failed to ensure water hose and air hose were not kept on the floor while employees were working/walking in the area.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) Mechanic Shop: On or about November 10, 2025, and at times prior, the employer exposed employees to struck-by and crushed-by hazards in that employer failed to ensure that energy isolating and LO/TO devices were located and utilized for Forward Lift four-post automotive lift while required repairs.
