29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee. Location - 78 Purchase Street, Newburyport, MA: On or about 11/2/25, an employee performing roof-related work approximately 20 feet above a lower level was exposed to fall hazards while using a personal fall arrest systems that was not adequately secured (tied off) to an anchorage, and/or was tied off to an anchorage point that was not capable of supporting at least 5,000 pounds per employee attached.
