29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about 10/10/2025 in the woodshop, when an employee used the (green) table saw Powermatic #66 to cut slats onto 3/4-in thick plywood, the employee suffered multiple finger amputation when they were not effectively protected from having their hands in close proximity to the blade while holding the jib steady to cult the slats. b) On or about 11/20/2025 in the woodshop, when employees operate the (black) table Saw to cut wood for shipping containers, employees are exposed to amputation hazards due to the table saw not having the guard in place while in use. NO ABATEMENT CERTIFICATION REQUIRED
