29 CFR 1910.147(f)(3)(ii)(D):Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment. On or about November 19, 2025, an an employee did not affix a personal lockout device to the Line 1 Carding Machine before accessing machine to perform maintenance. Repeat: NC Works was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(f)(3)(ii)(D), which was contained in OSHA inspection number 1790778, citation number 1, item number 2 and was affirmed as a final order on March 25, 2025, with respect to a workplace located at 16711 Applewhite Rd., San Antonio, TX 78264.
