Instance A: On or about February 28, 2026, the carpet lamination machine employees utilized had an unguarded ingoing nip point and rotating part hazard located on the south side of carpet alignment work area. Instance B: On or about February 28, 2026, the carpet lamination machine employees utilized had an unguarded ingoing nip point and rotating part hazard located on the north side of carpet alignment work area.
