29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Worksite: South Shore University Hospital Connector Building 301 E. Main Street, Bayshore, NY 11706 - An employee operating a Makita Circular Saw to rip 2in x 4in lumber, was not trained in the safe use of the saw. The employee was using one hand to hold the material being cut and one hand on the saw which resulted in a fingertip amputation; on or about November 14, 2025. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
