On or about October 10, 2025, the employer did not ensure that special hand tools were provided or used to prevent employees from placing their hands in the danger zone while adjusting a conveyor belt through a slitter machine (MO 203-4-99/72-20K/SN 2120) located at 4143 Rider Trail North, in Earth City MO. An employee sustained a crushing injury to his right middle finger when it was caught between a roller bar and table/conveyor belt while guiding the conveyor belt through the slitter machine.
