29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated  a) 715 Commerce Way, Billings, MT:  On or about November 21, 2025, and at times prior, employees were exposed to catapult and subsequent fall hazards of approximately 25 feet to the ground below while operating an Genie S-85 XC Aerial Lift without being tied off to the basket.

29 CFR  1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.   a) 715 Commerce Way, Billings MT:  on or about November 21, 2025, and at times prior, employees were exposed to fall hazards of approximately 25 feet to the ground below while kneeling on the leading edge of a steel structure installing roofing edge flashing onto the building.
