OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  a) 5 Parkway Circle New Castle, De 19720 (Shop bay): Employees were exposed to shear and crush hazards while removing the guard from the Scag Power Equipment Truck Loader leaf vacuum to service the machine. On November 20, 2025, an employee removing the guard removed the bolts in the incorrect order causing the guard to swing resulting in an avulsion to the right index finger and an amputation to the right ring finger.  Feasible recommended means of abatement  include but are not limited to:  1. Engineering Controls Install a Guard Retention System: Use hinged or captive fasteners so guards cannot swing freely when bolts are loosened. Energy Isolation: Implement lockout/tagout (LOTO) procedures before any maintenance. Ensure the impeller and moving parts are fully de-energized. Secondary Support Brackets: Add brackets or chains to hold the guard securely during removal.  2. Administrative Controls Written SOPs: Develop step-by-step procedures for safe guard removal, including correct bolt sequence. Training: Provide mandatory training on servicing equipment, emphasizing pinch-point and crush hazards. Job Hazard Analysis (JHA): Conduct JHA for all servicing tasks and review annually.  3. Personal Protective Equipment (PPE) Cut-Resistant Gloves: ANSI-rated gloves to reduce laceration risk. Eye and Hand Protection: Safety glasses and reinforced gloves during maintenance.  4. Manufacturer Guidance Follow Scag Power Equipment's service manual for guard removal and maintenance.
