29 CFR 1910.28(b)(12)(i): The employer did not ensure that each employee on a scaffold is protected from falling in accordance 29 CFR part 1926, subpart L:   On or about November 24, 2025, the employer failed to ensure that employees who were working from the scissor lifts (ESC 1932) were protected from a fall from height hazard as the scissor lifts were being operated with the gate door tied in the open position while in use.
