29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired 269 Farmington Avenue, Plainville, CT 06062: On or about November 24, 2025, Employer allowed Four Employees to utilize a portable ladder that showed signs of structural damage & defects that were not identified, repaired, or the ladder withdrawn from service. A 24-foot extendable ladder with visible impact damage on the side rails & rungs was left in service and operated by Employees at inspection.
