29 CFR 1910.178(l)(3)(ii)(G):Operator training did not include workplace related topics such as ramps and other sloped surfaces that could affect the vehicle's stability: a) Old Warehouse - On or about September 30, 2025, an employee was assigned to operate a Toyota Forklift, Model 8FD35U, Serial Number 20451, a powered industrial truck, to load packaged materials. The employee had not received training on the safe operation of powered industrial trucks on sloped surfaces (ramps), which could affect vehicle stability. Due to the lack of required training, the employee was exposed to struck-by and crushing hazards while operating the forklift on an inclined surface. This exposure resulted in a serious injury, including the amputation of the employee's left leg.
