29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) On or about November 25, 2025, an employee operating the Alligator Shear in the 180 Warehouse was exposed to a caught in hazard where the upper and lower portion of the shear meet. The employer did not ensure the point of operation was guarded to prevent any part of the body being in the danger zone or operating cycle.
