29 CFR  1910.147(d):  Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):  a. On or about November 26, 2025, at the East Pointe Bakery Case Former/Taper area location of the facility an employee was exposed to caught-in hazards when they performed maintenance activities such as but not limited to the 3M-Matic, Case Sealing Systems taper machine to remove the sheared bolt at the tape carriage for the tape machine. The employer did not ensure that the authorized employee was knowledgeable on the type and magnitude of the energy to be controlled, the hazards of the energy to be controlled and the means to control the energy prior to performing the maintenance work [per the 1910.147(d)(1) requirements].  The employer failed to implement energy control application steps concluding machine shut down to perform the servicing work.  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to protect the employee from the taper machine maintenance and servicing hazards.  The condition exposed employees to struck-by hazards as a result.
