29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a. On or about December 31, 2025, the employer did not ensure that the employee who performed the dough unjamming activities on the Kraiser Imprinter Roller machine were knowledgeable on the type and magnitude of the energy to be controlled on the machines, the hazards related to the energy to be controlled, and the means to control the energy, prior to performing the unjamming activities. As a result, employees were exposed to caught-in hazards.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence as required by 29 CFR 1910.147(d)(1) through (d)(6). a. On or about December 31, 2025, employees were exposed to machine hazards associated with the rotating roller when they performed dough unjamming activities on the Kraiser Imprinter Roller machine. The employer failed to implement energy control application steps to ensure the machine was shut down to perform the unjamming task [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to protect the employee from being caught in the machine.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. On or about December 31, 2025, employees were exposed to caught-in hazards at the Kraiser Imprinter Roller machine each time they worked near the rotating parts of the imprinter roller that was not guarded.
