29 CFR  1910.147(c)(6)(i):The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or about November 18, 2025,  the employer did not conduct a periodic inspection of the energy?control procedures for the element?line plasma assembly. Employees were performing maintenance on the plasma section, including replacing air cylinders, shock absorbers, and related components, without any documented annual review of the lockout/tagout (LOTO) procedures.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(f)(1):Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the sequence of actions in this section shall be followed.  On or about November 18, 2025, the employer did not ensure that employees followed the required safe?testing sequence for energized activation of pneumatic air cylinders within the plasma assembly of the element line.  An employee was positioned at the point of operation while testing the air cylinders and received a fingertip amputation.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
