29 CFR 1910.184(c)(9): All employees shall be kept clear of loads about to be lifted and of suspended loads. On or about September 5, 2025, the employer failed to ensure that employees were kept clear of loads about to be lifted and of suspended loads. Employees working in the Finishing Department operate "Crane 9: 2 Ton Stahl Drop Hoist Ship 1" to inspect and lower Industrial Cooling Radiators in excess of 2,000 pounds. Employees stood directly below and directly next to the radiators to move and spin them by hand for inspection, putting them in the fall zone (danger zone), exposing them to crushing and struck-by hazards which could result in permanent disability and death. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.184(f)(1): Sling use. Employers must use only wire-rope slings that have permanently affixed, and legible identification markings as prescribed by the manufacturer, and that indicate the recommended safe working load for the type(s) of hitch(es) used, the angle upon which it is based, and the number of legs if more than one. On or about September 5, 2025, the employer did not ensure wire-rope slings had permanently affixed, and legible identification markings as prescribed by the manufacturer, and that indicate the recommended safe working load for the type(s) of hitch(es) used, the angle upon which it is based, and the number of legs if more than one. (a) In the Ship1 Drop Area: Employees were exposed to struck by and crushed by hazards in that the employer allowed the employees to use wire rope slings without legible identification markings as prescribed by the manufacturer that indicated the recommended safe working load of the sling(s). Struck by and crushed by suspended industrial cooling radiators in excess of 2,000 pounds could result in severe injuries, permanent disability, and death. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. On or about September 5, 2025, the employer failed to ensure that electrical equipment was free from recognized hazards likely to cause death or serious physical harm to employees. The "Crane 9: 2 Ton Stahl Drop Hoist Ship 1" consists of two hoists that were not lifting and lowering at the same speed. This created a hazardous situation due to the spreader bar (lifting attachment) being unlevel as the employee(s) lowered industrial radiators weighing in excess of 2,500 pounds, exposing employees to crushing and struck-by hazards which could result in permanent disability and death. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
