FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

(a) Bagging Area: Energy isolating, blocking or securing devices were not applied to the pneumatic and electrical energy sources by employees who perform daily maintenance tasks on Hayssen Baggers including, but not limited to Model Ultima 12-16HP, Serial Number M334X82191-2, exposing employees to amputation hazards, on or about 12/4/25. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Throughout Facility: Annual audits of the lock out tag out procedures used by machine operators and maintenance employees when setting up and maintaining equipment including, but not limited to, the Hayssen Baggers, were not conducted on or about 12/4/25. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
