Assigned storage spaces were not located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons: On or about December 4, 2025, Monro Muffler and Brake Inc. DB Tire Choice Auto Service Centers located at 7801 Broadview Rd, Seven Hills, OH 44131, did not ensure compressed gas cylinders to include but not limited to Oxygen, Acetylene and C25 gas, were chained or secured from being knocked over.

29 CFR 1910.253(b)(4)(i):Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire; On or about December 4, 2025, Monro Muffler and Brake Inc. DB Tire Choice Auto Service Centers located at 7801 Broadview Rd, Seven Hills, OH 44131, did not ensure that the Oxygen compressed cylinders located within the workplace were being stored away from highly combustible material likely to cause or accelerate a fire. The Oxygen compressed cylinders were being stored with Acetylene fuel-gas cylinders.
