29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun:    On top of a structure located at 1100 St. Johns Bluff Road, N. Jacksonville, FL: On or about December 5, 2025, the employer exposed employees to struck-by hazards, in that employees were not wearing safety glasses while using pneumatic nail guns while conducting re-roofing operations.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a. On top of a structure located at 1100 St. Johns Bluff Road, N. Jacksonville, FL: On or about December 5, 2025, the employer exposed employees to a 17-foot 5-inch fall hazard, in that fall protection was not utilized by employees performing re-roofing operations on the upper level.  b. On top of a structure located at 1100 St. Johns Bluff Road, N. Jacksonville, FL: On or about December 5, 2025, the employer exposed employees to a 10-foot 1-inch fall hazard, in that fall protection was not utilized by employees performing re-roofing operations on the lower level.
