29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun:  On top of a structure located at 12763 Ashbrooke Circle W. Jacksonville, FL: On or about April 27, 2026, the employer exposed employees to a struck-by hazard, in that an employee was not wearing safety glasses while using a pneumatic nail gun while conducting re-roofing operations.  The Polaris Roofing Solutions, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1): which was contained in OSHA inspection number 1862706, citation number 01, item number 001 and was affirmed as a final order on January 8, 2026, with respect to a workplace located at 1100 St. Johns Bluff Road, N. Jacksonville, FL.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a. On top of a structure located at 12763 Ashbrooke Circle W. Jacksonville, FL:  On or about April 27, 2026, the employer exposed employees to a 15-foot 8-inch fall hazard, in that fall protection was not utilized by employees performing re-roofing operations on the upper level of a structure.   b.  On top of a structure located at 12763 Ashbrooke Circle W. Jacksonville, FL:  On or about April 27, 2026, the employer exposed employees to an 8-foot 6-inch fall hazard, in that fall protection was not utilized by employees performing re-roofing operations on the lower level of a structure.   The Polaris Roofing Solutions, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13): which was contained in OSHA inspection number 1862706, citation number 01, item number 002 and was affirmed as a final order on January 8, 2026, with respect to a workplace located at 1100 St. Johns Bluff Road, N.
