29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:  a- 117 Rutland Way, Woolwich Township NJ - A worker was exposed to fall hazards, in excess of 18 feet 1 inch to the ground surface below, when performing siding installation and other related activities from a metal pick that was improperly installed onto an existing pump jack's walking and working surface and extended in excess of 6 feet 9 inches beyond/over the pump jack scaffold's support. Condition observed on or about 12/04/25.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.451(b)(10): Scaffold components manufactured by different manufacturers were modified in order to intermix them and a competent person did not determine that the resulting scaffold was structurally sound:  a- 117 Rutland Way, Woolwich Township NJ - A worker was exposed to fall hazards, in excess of 18 feet 1 inch to the ground surface below, when performing siding installation and other related activities from a metal pick that was improperly installed onto an existing pump jack's walking and working surface and extended in excess of 6 feet 9 inches beyond/over the pump jack scaffold's support. Condition observed on or about 12/04/25.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a- 117 Rutland Way, Woolwich Township NJ - A worker was exposed to fall hazards, in excess of 18 feet 1 inch to the ground surface below, when performing siding installation and other related activities from a pump jack scaffold that was missing end stops and a mid rail/screen running along the scaffold's open sides and ends.  Condition observed on or about 12/04/25.   JFP Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(i), which was contained in OSHA inspection number 1823487, citation number 1, item number 1 and was affirmed as a final order on July 25, 2025, with respect to a workplace located at 232 Merseyside Drive, Woolwich Township NJ.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a- 117 Rutland Way, Woolwich Township NJ - A company worker, working from a deficient pump jack scaffold that he had erected at the worksite, had not been trained by a qualified person in order to recognize and minimize scaffold hazards.  Condition noted on or about 12/04/25.   JFP Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.454(a), which was contained in OSHA inspection number 1823487, citation number 1, item number 2 and was affirmed as a final order on July 25, 2025, with respect to a workplace located at 232 Merseyside Drive, Woolwich Township NJ.
