29 CFR  1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  On or about December 9, 2025, at a work location located at 13826 W. 130TH ST, Strongsville Ohio 44136 the employer did not ensure that a ladder used for access/egress to the roof of a commercial structure extended at least 3 feet beyond the upper landing surface and/or was secured at the top with a grasping device, exposing employees to falls over 16.2 feet.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   On or about December 9, 2025, during the installation of roofing material at a job site located at 13826 W. 130TH ST, Strongsville Ohio 44136. The employer did not ensure employees working on the roof of a commercial building were protected from fall hazards greater than 16.2 feet above the ground.   RM Building LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Fall protection), which was contained in OSHA inspection number 1621811, citation number 1, item number 1c and was affirmed as a final order on 2/3/2023, with respect to a workplace located at Northwest corner of PA-56 and Elm ST, Seward PA 15954.
