29 CFR 1910.132(d)(2):The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2). Building 1164: On or about December 18, 2025, and at times prior to, the employees conducted various maintenance tasks and cleaning tasks on the following areas on a regular basis without having a written hazard assessment established: a) Maintenance department b) Housekeeping department This condition potentially exposed employees to physical and chemical hazards. Notice 1, Item 1, 29 CFR 1910.132(d)(2): The employer failed to verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29CFR 1910.132(d)(2). The employer was cited for a violation of the same standard on November 16, 2023, inspection # 1707422, with final order date January 04, 2024, and means of abatement.

Telephone/Chemical Room: On or about December 8, 2025, and at times prior to, the eyewash station was partially blocked which prevented employees from immediate access for emergency use. This condition potentially exposed employees to chemical hazards. Notice 1, Item 1, 29 CFR 1910.151(c): The employer failed to clean and maintain suitable facilities for quick drenching or flushing of the eyes and body within the work area for immediate emergency use. The employer was cited for a violation of the same standard on February 26, 2025, Inspection 1801077, with final order date of April 7, 2025 and means of abatement.
