29 CFR 1904.29(b)(1): The employer did not enter information about the business at the top of the OSHA 300 Log, a one or two line description for each recordable injury or illness, and a summary of this information on the OSHA 300-A at the end of the year. a) 665 Independence Avenue, Unit A, Mechanicsburg, PA 17055 - On or about November 24, 2025, column F for the case no. 6 on the 2025 OSHA Form 300, Injury and Illness Log, provided the injury and part of the body affected, but did not include the object/substance that directly injured the person. b) 665 Independence Avenue, Unit A, Mechanicsburg, PA 17055 - On or about December 1, 2025, column F for the case no. 7 on the 2025 OSHA Form 300, Injury and Illness Log, provided the injury and part of the body affected, but did not include the object/substance that directly injured the person. c) 665 Independence Avenue, Unit A, Mechanicsburg, PA 17055 - On or about December 11, 2025, the 2025 OSHA Form 300 Injury and Illness Log did not include information about the business at the top of the form. Abatement certification required within 10 days after abatement date. 29 CFR 1910.1026(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI): a) 665 Independence Avenue, Unit A, Mechanicsburg, PA 17055 - On or about November 24, 2025, employees utilized stainless steel parts during welding operations and the employer did not determine the 8-hour-time-weighted average (TWA) exposure for each employee exposed to chromium (VI). Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
