29 CFR  1910.132(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever where it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  Site: On or about December 11, 2025, the employer failed to ensure employees used appropriate personal protective equipment to prevent skin exposure to hazardous chemicals during routine production tasks. The neoprene gloves worn by an employee while handling a compound containing Vanlink 1 PDR were of insufficient length to protect exposed forearms/upper arms which resulted in a chemical skin reaction.

29 CFR  1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or are likely to be present, which necessitated the use of personal protective equipment (PPE):  Site: On or about December 11, 2025, the employer had not adequately assessed the workplace to determine if hazards were present, or likely to be present, to ensure the use of personal protection equipment, including the protection against exposure hazards associated with chemical mixing operations involving  Vanlink 1 PDR such that inadequately sized hand and arm protection was not identified.
