29 CFR  1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  a) On or about November 20, 2025, and at times prior, employees in the Lamination Department were exposed to pinch points and amputation hazards due to an unguarded sprocket and chain within Tower 2 of the book loading. The employer did not ensure the chain and sprocket wheels used for book transfer were enclosed (guarded). There was a 7.5 inch tall opening to load books with a chain and sprocket inside. The chain and sprocket wheel were less than seven feet above the shop floor and horizontally within 4.5 inches of where workers' hands can access the opening.
