29 CFR  1926.20(b)(2):Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On or about December 16, 2025, at an establishment located at 5128 Achates Court in Ashwaubenon, Wisconsin, employees of Becks Quality Cabinets, Inc., working as subcontractors of M. Gerhard Construction, Inc. were exposed to fall hazards of over 10 feet 5 inches above the first floor and the employer failed to take prompt corrective measures to eliminate the hazards.

29 CFR  1926.502(a)(2):Employers shall provide and install all fall protection systems required by this subpart for an employee, and shall comply with all other pertinent requirements of this subpart before that employee begins the work that necessitates the fall protection.  On or about December 16, 2025, at 5128 Achates Court in Ashwaubenon, Wisconsin, employees of Becks Quality Cabinets, Inc., working as subcontractors of M. Gerhard Construction, Inc. were exposed to a potential fall hazard while working on a second floor mezzanine without a form of conventional fall protection. Second floor was approximately 10 feet 5 inches above the first floor.

29 CFR 1926.1052(c)(1)(i): Stairways. Stair-rails and handrails. The following requirements apply to all stairways. Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, shall be equipped with at least one handrail.   On or about December 16, 2025, at 5128 Achates Court in Ashwaubenon, Wisconsin, employees were exposed to a potential fall hazard using an access/egress stairway between a first floor and a second floor without a handrail installed. Second floor was approximately 10 feet 5inches feet above the first floor.
