29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance: (a) On or before December 16, 2025, the employer did not ensure that employees were protected from injury, in that employees were accessing and egressing the two story, multi family residential building via a metal extension ladder. Employees were accessing and egressing the ladder with tools and materials in their hands. This condition exposed the employee to a fall in excess of 6 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or before December 16, 2025, the employer did not ensure that employees were protected from injury, in that employees the employees were not utilizing a fall protection system during roofing work. This condition exposed employees to falls in excess of 6 feet. Blue Planet Metal, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1714791, citation number 1, item number 1, and was affirmed as a final order on February 09, 2024, with respect to a workplace located at 676 Mason Street, ERIE, CO 80516.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or before December 16, 2025 the employer did not ensure that employees were protected from injury, in that employees were accessing and egressing the two story, multi family residential building via a metal extension ladder that was extended to the gutter. The ladder was not secured and only extended 2 rungs or 24 inches above landing. This condition exposed the employee to a fall in excess of 6 feet.
