WAREHOUSE WORKSTATIONS:   On or about December 17, 2025, multiple series-connected relocatable power taps (RPTs) at warehouse workstations were used to energize computers, monitors, printers, and digital scales.  Relocatable power taps were not used in accordance with its listing/labeling and multiple series-connected RPTs is not an approved use for a relocatable power tap as stated in the UL Whitebook.

Warehouse - Sewing Department:    On, or about, September 22, 2025, the employer did not have a safety data sheet for the Topbond All Purpose Adhesive used in the sewing department.  Hazard pictograms on the container indicate that the contents are flammable, a health hazard, and an irritant or may cause narcotic effects.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  Warehouse - Sewing Department:    On, or about, September 22, 2025, the employer had not provided training to employees regarding the Hazard Communication standard and the safe use of the handling of hazardous materials such as Topbond All Purpose Adhesive used by employees to adhere patches on an order of personalized leather work gloves. Hazard pictograms on the container indicate that the contents are flammable, a health hazard, and an irritant or may cause narcotic effects.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  Warehouse - Kitting/Sewing Area:   On or about December 17, 2025, a portable fire extinguisher was blocked with cardboard boxes making the extinguisher inaccessible and subjecting employees to prolonged fire exposure and the byproducts of combustion.  3PL Worldwide, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1747860, citation number 1, item number 2 and was affirmed as a final order on September 18, 2024, with respect to a workplace located at 75 Aircraft Road, Southington, Connecticut.

Warehouse - Kitting/Sewing Area:   On or about December 17, 2025 the working space in front of the electrical panel was obstructed by a table and a chair.  3PL Worldwide, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1747860, citation number 1, item number 3 and was affirmed as a final order on September 18, 2024, with respect to a workplace located at 75 Aircraft Road, Southington, Connecticut.
