29 CFR  1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  (c)(4)(ii)(A) A specific statement of the intended use of the procedure; (c)(4)(ii)(B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (c)(4)(ii)(C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (c)(4)(ii)(D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  a) On or about December 19, 2025, employees performed service and maintenance on the up-cut saw to include blade changes without having procedures that provide the necessary specific information as required for the control of hazardous energy. The employer relied on a deficient energy control procedures for the up-cut saw, labeled Chop Saw, which exposed the employees to the hazards associated with performing service and maintenance; such as caught-in hazards. The procedure did not include the following:  (1) Specific scope and purpose statement to include, but not limited to blade changes and general maintenance of the saw -1910.147(c (4)(ii)[A]. (2) Clear and specific steps for shutdown, isolating, and securing the saw to control the electrical, gravitational, and pneumatic energy - 1910.147(c)(4)(ii)[B]. (3) Specific steps for placement, removal and transfer of lockout devices and the responsibilities of performing them on the saw - 1910.147(c)(4)(ii)[C].  (4) Specific requirements for testing the machine to verify the effectiveness of the lockout and energy control measures of the saw - 1910.147(c)(4)(ii)[D].

29 CFR  1910.213(r)(4):The mention of specific machines in paragraphs (a) thru (q) and this paragraph (r) of this section, inclusive, is not intended to exclude other woodworking machines from the requirement that suitable guards and exhaust hoods be provided to reduce to a minimum the hazard due to the point of operation of such machines.  a) On or about December 19, 2025, the employer did not ensure that the point of operation of the green up-cut saw, known as the chop saw, was adequately guarded. Employees were using the saw to cut approximately 1" x 3.5" boards with an unguarded opening of approximately 4" x 20" which exposed employees to a caught-in hazard.
