29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Excavation located at 3973 Canton Hwy., Cumming, GA 30040: On or about 01/06/2026, employees were exposed to cave-in and engulfment hazards while working inside a trench measuring approximately 5 feet 7 inches deep, 8 feet 11 inches wide, and 45 feet long, in that the employer did not provide a safe means of egress so employees could not exit quickly in an emergency, which could result in being trapped or crushed by a trench wall collapse.
