On or about January 7, 2026, in the 7.5-feet deep excavation, the employer did not provide a stairway, ladder, ramp or other safe means of egress, exposing employees to emergency egress hazards.

Location A Publix on 5000 SR-16, Saint Augustine, Florida. On or about January 7, 2026, in the 6-feet deep excavation, the employer exposed employees to struck-by hazards and engulfment hazards from unstable soil spoil piles placed at the top leading edge of the sidewalls of the excavation with excavated soil and debris sliding back into the trench. b. Location A Publix on 5000 SR-16, Saint Augustine, Florida. On or about January 7, 2026, in the 7.5-feet deep excavation, the employer exposed employees to struck-by hazards and engulfment hazards from unstable soil spoil piles placed at the top leading edge of the sidewalls of the excavation with excavated soil and debris sliding back into the trench.
