29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) Inside second level large room: Employees assembling a metal form for a balconette were working on a wooden scaffolding whose platform had an approximate 1 foot opening where there was a missing plank, right as they accessed the wooden scaffolding by climbing the wooden job-made ladder. Employees were exposed to fall hazards.
