29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Fireplace area, finishing presses; On or about January 8, 2026 and dates prior, the written energy isolation procedures for controlling electrical, pneumatic, and stored (gravity) energy were not utilized while employees performed servicing activities of setup/changeover operations on double-sided finishing presses (FIN 004 and FIN 008). Rather, operators used an unwritten procedure that relied on turning a rotary switch to the off position, so the material/shuttle table they were setting up would not move beneath the ram while the electrical, pneumatic, and stored energy for the ram and shuttle table remained energized.
