29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  On or about January 7, 2026, an employee was exposed to fall hazards greater than 10 feet, and the employee's fall arrest system was attached at the front of the employee's waist instead of in the center of the employee's back, on a residential construction site in Mahomet, Illinois.

LINING EXTERIOR & CONSTRUCTION, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1769433, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 30, 2024, WITH RESPECT TO A WORKPLACE LOCATED AT 206 MONROE STREET, SAINT JOSEPH, ILLINOIS 61873.  LINING EXTERIOR & CONSTRUCTION, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1749676, CITATION NUMBER 1, ITEM NUMBER 3a AND WAS AFFIRMED AS A FINAL ORDER ON JULY 10, 2024, WITH RESPECT TO A WORKPLACE LOCATED AT 708 S. AMY DRIVE, MAHOMET, ILLINOIS 61853.
