29 CFR  1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  a) On January 6, 2026, at the above addressed jobsite, the wood carpenters scaffold that was provided to employees performing carpentry work approximately 7 feet above the concrete floor was provided with walking platforms that were approximately 6 inches wide, exposing the employee to falls.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates and mud sills or other adequate firm foundation:  a) On January 6, 2026, at the above addressed jobsite, the wood carpenters scaffold that was provided to employees performing carpentry work approximately 7 feet above the concrete floor, was not erected with level, sound and rigid footing.  The footings on numerous supports were not secure or provided with base plates, thereby exposing the employee to falls.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a) On January 6, 2026, at the above addressed jobsite, employees engaged in framing activities were exposed to falls of approximately 10 feet 4 inches without the use of fall protection.  Employees were exposed to interior falls from the top of trusses to the interior floor.    b) On January 6, 2026, at the above addressed jobsite, employees engaged in framing activities were exposed to falls of approximately 11 feet 4 inches without the use of fall protection.  Employees were exposed to exterior falls from the top of trusses to the exterior ground on the north, west, and southwest sides of the area of the house being constructed.  Employees were exposed to exterior falls from the top of a trusses to the ground.    c) On January 6, 2026, at the above addressed jobsite, employees engaged in framing activities were exposed to falls of up to approximately 18 feet 10 inches without the use of fall protection.  Employees were exposed to exterior falls from the top of trusses to the exterior ground on the south side of the area of the house being constructed.  Employees were exposed to exterior falls from the top of a trusses to the ground.    No abatement certification or documentation is required for this item.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On January 6, 2026, at the above addressed jobsite, the portable Werner Fiberglass A-frame ladder used by an employee to access and egress the middle of the roof where trusses were being set, measuring approximately 10 feet 4 inches from the ground, did not extend to have the side rails extend at least three (3) feet above the upper landing.  b) On January 6, 2026, at the above addressed jobsite, the portable Gorilla Ladder used by an employee to access and egress the carpenters scaffold on the south wall of the home, measuring approximately 6 feet 6 inches from the ground, did not extend to have the side rails extend at least three (3) feet above the upper landing.   c) On January 6, 2026, at the above addressed jobsite, the portable Werner Fiberglass A-frame ladder used by an employee to access and egress the carpenters scaffold on the north wall of the home, measuring approximately 6 feet 11 inches from the ground, did not extend to have the side rails extend at least three (3) feet above the upper landing.   No abatement certification or documentation is required for this item.
