29 CFR  1910.140(c)(6): A competent person or qualified person must inspect each knot in a lanyard or vertical lifeline to ensure that it meets the requirements of paragraphs (c)(4) and (c)(5) before any employee uses the lanyard or lifeline.  a) On or about January 9, 2026, and days prior, the employer did not ensure the knot in the Miller Mini Lite retractable lifeline was inspected prior to use on the Jungheinrich EKS208 FN01739 forklift to ensure it was capable of withstanding the required breaking strength and load. Picker operators were exposed to fall heights of greater than 15 feet while utilizing knotted and worn fall protection lanyards.  b) On or about January 9, 2026, and days prior, the employer did not ensure the knot in the Miller Scorpion Fall Limiter retractable lifeline was inspected prior to use on the Jungheinrich EKS208 FN501740 forklift to ensure it was capable of withstanding the required breaking strength and load. Picker operators were exposed to fall heights of up to 19 feet while utilizing knotted and worn fall protection lanyards.  c) On or about January 9, 2026, and days prior, the employer did not ensure the knot in the 6ft Miller Turbo Lite limiter retractable lifeline was inspected prior to use on the Jungheinrich EKS208 FN535378 forklift to ensure it was capable of withstanding the required breaking strength and load. Picker operators were exposed to fall heights of up to 19 feet while utilizing knotted and worn fall protection lanyards.

29 CFR  1910.140(c)(18): Personal fall protection systems must be inspected before initial use during each workshift for mildew, wear, damage, and other deterioration, and defective components must be removed from service.  a) On or about January 9, 2026, and days prior, the employer did not ensure fall protection systems were being inspected before initial use each workshift. Picker operators were exposed to fall heights of up to 19 feet while utilizing altered and worn fall protection lanyards which contained knots in the lifeline.

At the end of each calendar year, you must:   29 CFR 1904.32(a)(2): Create an annual summary of injuries and illnesses recorded on the OSHA 300 Log;  a) On or about February 1, 2026, the employer did not accurately complete the OSHA 300A annual summary based on 2025 data from the OSHA 300 log to include the correct number of injuries, total number of days away from work, restrictions, and certification date.
