29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:   Maintenance Lock Up:  On or about January 16th  2025, and at times prior to, the employer did not ensure that the oxygen and acetylene compressed gas cylinders were separated by a minimum distance of 20 feet, or by a noncombustible barrier at least 5 ft. high having a fire resistance rating of at least one hour. This condition potentially exposed employees to fire and explosive hazards.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.   Maintenance Room: On or about January 16th, 2025, and at times prior to, a relocatable power tap was "daisy chained" via extension cord to a second portable power tap. This condition potentially exposed employees to electrical hazards.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Main Building: On or about January 16th, 2026,  and at times prior to, an eye wash station was not maintained as required, in that the eyewash was not activated weekly to flush the system and ensure proper operation.  This condition potentially exposed employees to struck by hazards.
