The employer's energy control procedures did not clearly define and specifically outline the techniques utilizes to remove hazardous energy from the Wardwell braiders prior to changing yarn or bobbins and/or other servicing involving the equipment deck.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by:  On or about January 15, 2026, employees engaged in production activities in Area B of ContiTech USA, LLC located at 2701 West Omaha Avenue, Norfolk, Nebraska were exposed to amputation and caught in hazards.  The employer did not protect employees from contact with moving parts when entering the horizontal/wire braiders.  Specifically, the electrical lockout/tagout position for the MR11, MR15, and TMW horizontal and/or wire braiders was located inside of the equipment enclosure interlocked doors, which required employees to enter the enclosure door to shut off and lockout electrical in accordance with energy control procedures.
