29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about February 11, 2026, and at times prior, employees were exposed to amputation hazards, rotating parts, and flying chips from operating the Bridgeport Manual Mill in the West Building, 230 VAC. The manual mill was missing a shield or chip guard which protected employees from the moving drill bit. b) On or about February 11, 2026, and at times prior, employees were exposed to amputation and laceration hazards from operating the Select Vertical Band Saw in the West Building, Model# VBS14 Contour Band , Serial# 750005. The vertical band saw was missing a guard which protected employees from the moving saw blade. c) On or about February 11, 2026, and at times prior, employees were exposed to amputation hazards, rotating parts, and flying chips from operating the drill press in Casting in the East Building. The drill press was missing a shield or chip guard which protected employees from the moving drill bit. Abatement Note: Abatement certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) On or about February 11, 2026, and at times prior, employees were exposed to noise above the OSHA Action Level of 50% dose while operating Power Presses in the East Building and CNCs in the West Building. Two employees exceeded the OSHA Action Level of 50% dose at 95.5% and 71%. The employer failed to have an effective Hearing Conservation Program in that some of the employees in the East Building were wearing headphones and not ear protection as required.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: a) On or about February 11, 2026, and at times prior, press operators and helpers, CNC Mill operators, Casting operators, and maintenance employees were exposed to crushing hazards while performing machine operations and maintenance while utilizing machine specific lockout procedures. The procedures were inadequate to inform and protect workers in that they did not include the following: 1. Employee training 2. Periodic inspection requirements 3. How to transfer LOTO devices between coworkers, shifts, etc.
