29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  At the job site located at 209 S Sycamore St. Wichita, Kansas, employees engaged in cutting and welding activities for a 3' X 2' 6" roof hatch, were exposed to fall hazards of approximately 14 feet 6 inches from the hole opening to the first floor below.  Employees had their harness on, but were not tied off from the anchor point. An employee fell through the hole opening and hit the concrete ground below. The employee was hospitalized.
